Parents/ Guardian’s Affidavit
I am aware that my ward has to follow these rules at all times during his/her period of studies at the College of Social Work (Autonomous):
- Attend all classes i.e. Lectures, tutorials and field work practicum regularly and his/her attendance will be as per ordinances of University of Mumbai and academic rules of autonomy, failing which I understand that my son/daughter ( “my ward”) shall be detained and will not be allowed to appear in the examination
- In the event of bunking classes, showing poor attendance, committing any misconduct or breach of rules or discipline of the Institute, my ward shall abide to the decision of the Principal/ Controller of Examinations or his/her representative.
- My ward shall render himself/ herself liable for the award of such punishment as is deemed fit by the Principal/ Controller of Examinations or his/her representative for the commission of any offense, misconduct, breach of rules or discipline of the Institute. I fully realize that the punishment may even lead to expulsion from the Institute/cancellation of admission and the punishment so awarded shall be final and binding on my ward.
- My ward shall be expelled or rusticated if he/she is found (i) possessing or bringing any type of intoxicant to class or in and around 100 meters of college premises (ii) part of unlawful gathering inside institute. (iii) violating any of hostel rules (for inmates of hostel) (iv) involving in quarrel/fight with fellow students/seniors or calling outsiders inside the Institute campus (v) indulging in ragging (vi) roaming around in the Institute while classes are in full progress.
Declaration by Parent/Guardian

I shall advise my ward to follow above rules strictly and if my ward is found to violate any rule as mentioned above, Institute authorities have the right to take action against him/her including expelling from college and I shall have no representation to you or in any court of law or to any other Institute/University authorities.
